I googled it and Amazon has one for $29.95, including shipping. That being said, if I were in your shoes, I would not go that route. When it is time for my PSA test, I just call my PCP's office and ask them to fax a PSA request to the local Quest Diagnostics lab, where 100% of the cost is covered by my insurance. NOTE: you BC/BS guys out there know that, come the end of December, BC/BS will no longer cover the cost of work done by that lab. So, Chuck, if you are insured by Blue Cross, you can contact them for a list of the local labs that they do support.
Lastly, I don't know enough about your Urologist to pass judgement on him, but if you are unhappy, you might want to consider finding someone else. The Castle Connolly website, (which is a respected award, based on peer nominations, that recognizes the top doctors in their field), can help you out there.
You can order your own PSA test from an online company. I have used "Private MD" and they will send you to a LAB CORP lab to take your blood sample. $47.00
