A friend's experience

How far off can biopsies be? A friend was diagnosed with Pca, Gleason 6 on biopsy. Post surgery pathology showed 4+5. Has anyone seen anything like that? And we probably have the same pathologists. Should I be concerned? His biopsy did show much more widespread cancer than mine. He had a second opinion from Johns Hopkins too.
